·         Recuperative Care – Provides hospitals with a safe discharge option for individuals experiencing homelessness who are not sick enough to remain in the hospital, but too sick for a shelter. The program provides individuals with a safe, clean environment to recuperate, medical oversight, linkages to primary care physicians and intensive case management services  to thereby reduce their chances of relapse and costly re-hospitalization. Intensive case management focuses on linkages to permanent housing and permanent supportive housing for individuals.

·         Harold Cares About Your Future: Pregnant and Parenting Teen (PPT)  – A school-based teen pregnancy prevention program empowering pregnant teen mothers to avoid a subsequent pregnancy, develop a positive, healthy pregnancy, birth and parenting experience, and remain in school with the goal of graduation. Overarching, the program aims to empower teen parents to develop lifelong healthy behaviors for self and child and increase academic achievement.

·         Health Academy – A youth-driven afterschool program engaging youth in leadership, critical thinking, problem solving and community-based research to address obesity and obesity related illnesses. The program empowers youth to apply comprehensive evidence-based obesity prevention strategies and advance policy, systems and environmental changes to increase consumption of healthy foods, reduce consumption of less healthy foods and beverages, and increase levels of physical activity to thereby build a healthier community.
